It should not be possible to spread a virus from vitualbox to the host OS unless that virus is specifically coded to take advantage of a security hole in virtualbox.
It is possible for the OS running in virtualbox to get infected, so a virus scanner for that would not be a bad idea.
First I would recommend you run apache on a Linux guest OS.
That said, is this going to be a production Server? (i hope not) Chances are a firewall would give you more benefit then a virus scanner. But in the end it all comes down to how paranoid you are.
The guest OS (XP) will still be vulnerable to viruses. Even if you have an antivirus app installed on the host OS, it will not be able to scan and remove viruses from the guest OS. You should install an antivirus app on the guest OS — I recommend Microsoft Security Essentials or Avira AntiVir.
You can created a shared folder between the host and the guest OS … Place what all files that you need in guest OS in the folder, so that you can access them, and install it the usual way. The trick is to make it available in the guest OS. I too had used virtual box while writing this post , read it-
