Check your Linux firewall and Selinux settings.
Turn them both off and see if you can connect. If you can connect with them turned off, you know you need to configure them to allow access.
A 500 error is usually a syntax error. Look in the error log to see which line it’s occurring on. (Use FirePHP to see what $host, $user, $pass and $db are at runtime.)
